RESOLUTION OF THE CITY COUNCIL OF THE CITY OF MOORPARK DESIGNATING
THREE CITY OFFICIALS TO ACT ON BEHALF OF THE CITY FOR THE PURPOSE OF
COMPLETING CERTAIN FORMS TO RECEIVE STATE AND FEDERAL DISASTER
ASSISTANCE
WHEREAS, when a State or Federal disaster is declared and the City is within the geographic
boundaries of said disaster area and incurs costs while responding to these emergencies, State and
Federal assistance may become available to help pay for this expense; and
WHEREAS, in order to receive said State and/or Federal assistance, the City must execute
certain forms, contracts and abide by certain assurances; and
WHEREAS, the State of California has developed a universal form which identifies by
position certain City officials to represent the City for the purpose of filing certain forms to receive
disaster assistance;
NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF MOORPARK DOES RESOLVE
AS FOLLOWS:
The State Resolution provided in Exhibit "A" to this Resolution, be completed and submitted
to the State Office of Emergency Services, designating three positions in the City, to act in the order
presented on behalf of the City and execute certain forms for the purpose of receiving disaster
assistance from State and Federal resources;

penalty of perjury that the foregoing Resolution No. 98 -1448 was adopted by the City Council of the
City of Moorpark at a meeting held on the 18th day of March, 1998, and that the same was adopted
by the following vote:
AYES: Councilmembers Evans, Perez, Teasley, Wozniak and Mayor Hunter
NOES: None

ABSTAIN: None
