L. , Services Director PREPARED BY: Peggy Rothschild, Active Adult Center Supervisor DATE: October 5, 2007 (CC Meeting of October 17, 2007) SUBJECT: Consider Resolution Authorizing Submittal of a Grant Application for the "Strike Out Stroke" Program DISCUSSION The City Council is being asked to adopt a resolution (Attachment A) approving and authorizing the submittal of a grant application to Livingston Memorial Foundation for $9,000 in contract grant funds for the "Strike Out Stroke" Program. The Resolution also authorizes the City Manager to sign all grant related forms on behalf of the City. Staff is making this recommendation as current funding for this stroke education and prevention program through Ventura County Area Agency on Aging will end after the current fiscal year. The Grant application is due October 24. Since stroke is the third leading cause of death in the United Stations nationally, stroke education and prevention are of vital concern. Of the 750,000 strokes that occur in the US each year, 600,000 could be prevented. Stroke is of particular concern to older adults as stroke risk increases with age; for each decade after age 55, the risk of stroke doubles. Stroke bears a high cost for the individual and the community: 10% of stroke victims require care in a nursing home or other long-term care facility; 15% of stroke victims die shortly after a stroke; 40% of stroke victims experience impairment requiring special care. Livingston Memorial Foundation is accepting Applications for Grant Consideration for programs which provide health and medical welfare to the people in Ventura County. If awarded, the proposed grant funds will be used to provide free carotid artery testing at the Moorpark Active Adult Center to Ventura County residents age 60 and older. S:\Community Services\ADMINISTRATIVE\CC Reports\2007\10-17-07 (Livingston Strike Out Stroke).doc 000 U44 Honorable City Council October 17, 2007 Page 2 STAFF RECOMMENDATION Adopt Resolution No. 2007- Attachment: A - Resolution RESOLUTION NO. 2007- A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF MOORPARK, CALIFORNIA, AUTHORIZING A GRANT APPLICATION REQUESTING MONIES FROM LIVINGSTON MEMORIAL FOUNDATION FOR THE "STRIKE OUT STROKE" HEALTH SCREENING AND EDUCATION PROJECT THROUGH THE MOORPARK ACTIVE ADULT CENTER. WHEREAS, the City Council is dedicated to the support of beneficial programs for older Americans in the City of Moorpark; and WHEREAS, providing free health screening and education services to area seniors can help maintain health and wellness in our older population; and WHEREAS, funds for the "Strike Out Stroke" Health Screening and Education Project are available in the form of a grant through Livingston Memorial Foundation. NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF MOORPARK DOES HEREBY RESOLVE AS FOLLOWS: SECTION 1. The City Council hereby approves and authorizes the grant application through Livingston Memorial Foundation to secure funds for the "Strike Out Stroke" Health Screening and Education Project. Further, the City Council hereby authorizes Steven Kueny, City Manager, to serve as signatory for the City Council on all matters related to the administration of this grant. SECTION 2. The City Clerk shall certify to the adoption of the resolution and shall cause a certified resolution to be filed in the book of original resolutions.
