Equipment Company for the construction of miscellaneous work to replace sidewalk, curb and gutter, asphalt and driveway apron. The amount of the contract was $17,842.02. An additional construction contingency in the amount of $1,720.00 was also approved. [Total: $18,562.02] A. Scope of Work The scope of work for this construction contract is generally described as follows: 1. Sidewalk reconstruction at 17 locations. 2. Concrete curb and gutter reconstruction at 9 locations. 3. Driveway apron and asphalt reconstruction at Mountain Meadows. 4. Remove existing curb and gutter, and asphalt, and construct new curb on McFadden Avenue. 5. Construction of sidewalk improvements along the Walnut Street frontage of the Moorpark Community Church at the northeast corner of Charles Street and Walnut Street, funded by the Moorpark Redevelopment Agency. B. Completion Construction of the project began on May 21, 1997. Correction of all punch list items was completed on October 29, 1997. 0004.3 r-- -M ' Miscellaneous Concrete December 1, 1997 Page 2 C. Additional Work 1. Change Orders: Four construction change orders were issued. They are summarized as follows: (1) Thirteen additional work areas involving sidewalk and /or curb and gutter were added to the contract. (2) An irrigation line was added to enable watering of planter areas at the corner of Walnut and Charles Streets. (3) Saturday work was performed at Mountain Meadows School to avoid bus disruption. (4) A credit was applied to the project to cover City costs to clean up various work sites. D. Fiscal Impact 1. School District's Share: As mentioned above, the Scope of Work included reconstruction of a driveway at Mountain Meadows Elementary School. The School District's share of these costs was $2,486.72. The District has been billed for this amount. D. Project Acceptance The City Engineer has completed a final inspection of the project and has found the work tp be acceptable. 0004.33 Miscellaneous Concrete December 1, 1997 Page 4 RECOMMENDATION Staff recommends that the City Council take the following actions: Accept the work as complete, except as noted in this report; 2.
